APPLICATION FOR RESIDENCY

Submit by either:
1. Fill out online form and submit.
2. Saving completed form to your computer and then sending to us as an email attachment to info@graciouslybroken.com
3. Print a copy, complete form, and mail it to us at:

Intake Procedure:
● The Applicant will fill out an application and turn into the office.
● An interview with a senior staff or Director is required. This is to determine that the applicant meets the basic eligibility requirements and if they are willing to submit to program guidelines and policies.
● If the Applicant is deemed willing and eligible, then the application is turned over to the Case Manager for review.
● The application will be processed and a complete file will be assembled. The application, background checks and any other vital information will then be turned over to the Director for final approval.
● With final approval, the applicant is accepted and the intake process continues or, if space does not permit, is placed on a waiting list.
● If the applicant is not accepted, then the designated leader will inform the applicant. If an applicant is denied, we will try to recommend another program suited for his needs.
Applicant Criteria:
1. Identification papers and a background check are required.
2. At this time, applicants are not eligible to apply for admission who:
a. are registered sex-offenders
b. have major medical conditions that require a doctor’s care
c. have mental disorders that require a doctor’s care and/or medication
d. have violent tendencies. We must protect the safety of our staff and residents.
e. are coming directly out of prison. The applicant must wait 6 months after release before entering the program, if accepted.
3. If you are eligible, an interview will be conducted. We accept any woman who is willing to abide by and adhere to our rules, regulations and bylaws and live under the direction and guidance prescribed in the Bible.
4. Applicants must be clear headed upon entering the program.
5. All new admissions must go through a 30 day orientation period:
a. Physical exam and lab test, as needed
b. Learn routine, go over guidelines and procedures
c. No outside contact
d. Counseling / mentoring appointments
e. No recreational activities
6. Applicants’ belongings will be searched for drugs or anything that is harmful to your spirit, soul or body. Random searches will take place.
7. We must be made aware of any medications that applicants are currently taking.
8. We are not equipped to handle special dietary requirements at this time.
9. Due to space limitations, there is a limit on the amount of clothing and personal belongings that is allowed in the house.
10. Anything that is not approved or does not comply with the guidelines and procedures will be removed. Family members may pick up confiscated items within 7 days. Otherwise, items will be donated or disposed of.
11. All Identification documents, billfolds, cellphones, money, etc. must be turned into the office to be locked up. Items may be checked out, when necessary. Monies will be deposited for safekeeping and applicant will be provided with receipt. This is for your protection and ours.
